Pre-Application Request Form

Established in 1999 to eliminate duplicative credentialing for members of the Lexington Medical Society and local physicians. One application and one verfication, thus minimizing the administrative burden on the participating healthcare facility, the physician, and practitioner.

Commonwealth Credentialing also processes reappoinment applications for all participating facilities, thus further "lightening the load" for our members, local physicians and facilities.

    The annual Commowealth Credentialing Advisory Committee meeting, consisting of representatives from Lexington Clinic, Baptist Health Lexington, Kentucky OneHealth, UK Good Samaritan, Lexington Surgery Center, Kentucky Surgery Center, and Baptist Physician Surgery Center, meets to review the credentialling process and ensure it is in compliance with regulatory guidelines and continues to meet the needs of the facilities.
