How to Apply

Membership in the Greater Louisville Medical Society is open to MDs and DOs, including residents and fellows, as well as medical students from the University of Louisville. DMDs and PhDs are eligible for Associate membership.

  • If you have been a member of GLMS before, or if you have gone through the Centralized Application Processing Service (CAPS) at GLMS, e-mail membership@glms.org to request a pre-populated application. You will only need to cross through outdated information and write in the newest information, saving you time.

  • If you are currently working on a CAPS application for hospital privileges, your membership will be processed at the same time. Tell any CAPS staff person you'd also like to add GLMS membership.

  • If you are not a past member or CAPS client, apply by filling out one of the fillable PDFs below. When complete, e-mail to membership@glms.org, fax to (502) 736-6363 or mail to Membership Department, Greater Louisville Medical Society, 101 W. Chestnut St., Louisville, KY 40202.

 

   

 

  • If you are a medical student, please e-mail membership@glms.org to be added to our e-mail list. Your membership is complimentary.

 

The Application Process

1. Complete an application as described above and return to GLMS.

2. The GLMS credentialing specialists will verify the information provided on your application. This can take as long as a month.

3. After your application has been satisfactorily verified, you will be presented to the Judicial Council at the next membership election (held the 1st of each month).

4. After election, you will be notified by mail of your membership status. You will not be billed for your membership until after your application has been processed.

Note: The length of this process depends on how quickly your application verifications are returned to our staff. If your application has been processing longer than a month, please contact us to be sure it was successfully received.
